Xray/MRI Review

To determine if an office appointment would be beneficial for you, American Hip Institute physicians will review X-rays, MRIs, or CT scans of your hip, knees, or shoulders. To have your imaging reviewed, please send the following materials to the indicated address. Be sure to carefully read these instructions to ensure that your records are complete. Incomplete records are subject to delayed review until complete. If you would like your records to be returned, you must include a return envelope with postage within the submitted package.

STEP ONE: Submit Materials

  1. A completed Intake Form download here
  2. A signed Consent for Review of Records & Imaging Studies Form download here
  3. A copy of your insurance card
  4. Diagnostic imaging and reports: X-rays, MRI/Arthrograms, and CT scans, along with the reports that correspond to the imaging. Images should be on a disc.
  5. Office notes from previous/current treating physicians
  6. Operative reports and images specific to the affected joint.

STEP TWO: Complete Registration

A Care Specialist will contact you via phone when we have received your records to confirm receipt, complete your registration, and route your materials to the clinical team for review. They will ask you for some additional information needed to enter you into our medical record system.

STEP THREE: Review Phone Call

A member of the American Hip Institute clinical team will review your records with our physicians and call you directly via phone to discuss whether an office appointment would be beneficial for you. Please note that there is no charge for medical record reviews outside of Illinois, Wisconsin, and Indiana. Carefully review the disclaimer below to clearly understand the scope of this review.
